Referring Clinic Resources
Our doctors and staff realize that you have a choice when referring out your cases - we invite you to contact us if there is anything we can do in order to better serve you and your patients. We look forward to working with you.
Once you decide that you'd like to transfer a case to us, we begin by collecting information about the case. To do so, please select the appropriate transfer form below. These forms can be faxed to us (253-474-6057), or sent with the client. Of course, you are always welcome to call and speak directly with one of our doctors, who will record the case information. When your client arrives, a triage technician will assess the patient and begin immediate supportive care, if indicated. One of our ER doctors will examine the patient, speak with the client, and determine whether the case will be inpatient or outpatient.
We strive to maintain excellent communication with our referring clinicians. Records will be faxed to your facility upon discharge. For patients that are admitted to the hospital, you will also receive a daily case update via fax. As always, give us a call 24/7 whenever you want to check on a case. Patients are always referred back to you for follow-up care...after all, they are your patients!
